Is it possible to set up an Automation workflow that is triggered when a subscriber changes from one group to another?

Further Context: 

I work for a medical practice, which switches its newsletter recipients from Patients to Post-Treatment after their case is completed.

We'd like to send an automated request for online practice reviews to the Post-Treatment list, but I can't figure out how to set up a workflow for a switch from one group to another.

Any advice would be greatly appreciated.

Hey again! I chatted real quick with a product owner to see if he had any thoughts about a workaround. He suggested that you may be able to get what you're looking for by using a radio or drop-down field to capture status (like "Patients" or "Post-Treatment") and using a Field Change trigger to automate messages. This would only work if you have an API connection that would be able to update individual records, though.

So that might be an option for you in the meantime!

We don't have a team directly working on this right now, but can see how this would benefit you, so I'm marking this as Under Consideration.
