No surveys appearing in dropdown for SurveyMonkey

I've connected our client's SurveyMonkey account, but the dropdown is empty (and appears broken) when I try to select a survey to link to. I've double checked the SurveyMonkey account to ensure there are active surveys. I've tried this in both Chrome Version 50.0.2661.94 and Firefox 44.0.2 Any ideas?

Hey Steve, 

Can you point me to the sub-account you're seeing this in?  

Also, if you wanted to give a try disconnecting SurveyMonkey and reconnecting, you'll need to follow the directions I posted on THIS THREAD in order to disconnect.  

Also, have you tried using the SurveyMonkey block?  Are any surveys showing up there?  

I'll be watching out for your reply!

Did you wind up trying disconnecting and reconnecting?  Any luck?
